We understand that when you are in pain, you don’t want to wait weeks for an appointment. Our friendly office staff is happy to take your call Monday – Friday from 9:00am to 5:00pm. To expedite the appointment process, please have your insurance information ready.

Complete Registration Forms

Download the registration forms and complete them prior to your appointment. If you’re unable to complete the forms ahead of time, make sure to arrive 30 minutes prior to your scheduled appointment time.

Come Prepared for Your Appointment

It’s important that you bring these items to your first appointment:

  1. 1. Completed registration forms if you have them
  2. 2. Your driver’s license or other form of photo identification
  3. 3. Your insurance card
  4. 4. Any current MRIs, CT Scans or x-rays taken for your condition
  5. 5. Your co-payment if applicable (credit/debit card or cash only, we do not accept checks for co-pays)

What to Expect on Your First Visit

The first thing you will notice when you come to our office is the warm, comfortable environment. You will be greeted by our front office staff who will check you in for your visit.

GETTING INFORMATION ABOUT YOUR MEDICAL CONDITION

On your first visit, our clinic will:

  • Take your medical history
  • Perform an examination
  • Review any imaging studies including MRIs, CT Scans,
    or x-rays that you brought with you

IMPORTANCE OF IMAGING STUDIES

Dr. Bullard will evaluate your imaging studies to help diagnose your condition and determine the most appropriate treatment plan. No recommendation is made without imaging. If you don’t have any imaging or if additional workup is required, our staff can arrange this for you.

OUR TREATMENT APPROACH

Dr. Bullard believes the least invasive approach to a problem is probably the best. He will provide you with conservative treatment options first if it might help your medical condition. This may include lifestyle changes, physical therapy or pain management. When surgery is recommended, Dr. Bullard takes a minimally invasive approach that greatly reduces recovery time and requires only a short hospital stay.

Any treatment options that require you to see another provider will be arranged by our office. You will either receive a script to bring to the provider or our referral coordinator will contact them directly to make arrangements.

If it is determined that surgery is your best option, we will schedule it for you and arrange your pretest with the hospital. Prior to leaving our office, you will receive an information packet about the surgery you will be having. You may also meet with our insurance manager to review your coverage and we will obtain preauthorization for your surgery prior to your surgery date.
